Pick it if you want the whole complex at your own pace and you are sure of your dates, because the listing states it is non-refundable.

Four versions sit behind one booking button, and the choice matters more than it looks. Cathedral and Terraces by Stairs means about 250 steps up a spiral staircase, 251 by the most-quoted count. By Lift means a short ride from the entrance behind the cathedral, and the listing attaches fast-track access to the terraces and the cathedral to that version. Each comes with or without an audio guide in ten languages. Whichever you pick, the lift lands on the first terrace and the highest walk under the spires still needs about 80 steps on foot. Nobody rides to the top.
The ticket covers the nave, the terraces, the Duomo Museum at Piazza del Duomo 12 and the Church of San Gottardo in Corte behind the Palazzo Reale. It does not list the archaeological area under the parvis, so do not expect the baptistery. The voucher is valid for 48 hours from the booked date, each area can be entered once, and the listing gives 17:50 as the last entry everywhere. The museum and San Gottardo are closed on Wednesdays; a Wednesday booking means the museum half of the ticket has to wait until Thursday, which the two-day validity allows.
Two caveats before you click. The listing states the ticket is non-refundable, so a change of plan or a rainy day costs you the ticket, and the terraces close in rain, ice or strong wind. And a ticket does not skip the security check: everyone passes the same airport-style control in front of the facade. What the fast-track version skips is the ticket queue. Bring a passport or ID card, because the Fabbrica's tickets are nominal.
Stairs or lift, audio guide or not. The lift version sells out first in high season and is the one with fast-track access. Avoid a Wednesday if you want the museum on the same day; the two-day validity is your fallback.
Join the security line in front of the facade. The scan of the voucher happens here; there is no counter to visit. Shoulders and knees covered, no large bags, no cloakroom. Early morning is the shortest wait.
Allow about an hour. The meridian line runs across the floor by the entrance, St Bartholomew carries his own skin in the right transept, and the Holy Nail reliquary sits about 40 metres up in the apse vault. Mass is said six times a day on weekdays and visits pause around the altar during services.
Stairs on the north side of the cathedral, or the lift behind the apse, signposted. Both arrive on the first terrace. From there about 80 steps climb to the walk along the roof of the nave, where the Madonnina is about 40 metres above you.
The way down is by the stairs for most visitors, which is the easy direction. The Duomo Museum is in the Palazzo Reale at Piazza del Duomo 12, open from 10:00, closed on Wednesdays. San Gottardo is a few steps further, through the palace courtyard.
The voucher runs for 48 hours from the booked date, one entry per area. Many people do the cathedral and the roof on day one and the museum on day two, when the legs have recovered.
